Start with the task, then identify the official form

For Gurgaon District Court, the correct form depends on what you are trying to do: institute a civil case, upload an application through e-Filing, seek a certified copy, inspect a judicial record, file an appeal or revision, or complete another registry process. Use the relevant official court or eCourts source rather than copying a form from an unrelated court or old website. For local case-category and code guidance, see Gurgaon Case Types.

Open official civil filing form

Important: A downloadable form can be only one part of the filing. Pleadings, affidavits, annexures, court fee, signatures, verification, indexing and portal fields may be separately required. Forms and filing interfaces can change, so verify the current version from the official source before submission.

Which court form do you need for Gurgaon District Court?

The first distinction is between a case-filing form and the substantive legal document being filed. The official eCourts Civil Case Filing Form captures structured case information such as case type, party details, advocate details, lower-court information, main-matter information and office-use fields. It does not replace the plaint, petition, written statement, affidavit or other pleading required by the governing law and court rules.

The eCourts e-Filing service also operates as an online workflow rather than merely a library of printable forms. It allows advocates and litigants to file cases and applications, upload pleadings and documents in PDF form, e-sign documents and make online court-fee payments where the relevant integration is available.

Official eCourts filing form and online filing

The national eCourts service publishes a District & Sessions Court civil filing form. The form asks for the case type, plaintiff and respondent information, contact details, advocate details, lower-court details where relevant, and the main-matter details. Office fields include filing number, filing date, objection removal/compliance dates, registration number, listing date and court allocation.

That form is useful as a structured checklist even where the filing is completed through the electronic system. The e-Filing platform states that civil and criminal cases can be filed before District Courts and High Courts, and that litigants and advocates can upload pleadings/documents and submit applications online.

Punjab & Haryana High Court forms and proformas: when are they relevant?

The Punjab & Haryana High Court maintains an official Forms/Performas page containing, among other resources, opening sheets for civil appeals and civil revisions, inspection-branch forms, criminal revision and criminal appeal opening sheets, and current instructions relating to court fees and filing of certified copies.

Those materials should not automatically be treated as Gurgaon District Court filing forms. Some are designed for High Court proceedings or High Court administrative processes. They become relevant when the particular filing route or governing rule requires that format, or when a matter moves from the district judiciary to the High Court. The safest approach is to match the form to the receiving court and procedural task.

Forms for certified copies

A certified-copy request is a separate court-service workflow. The applicable copying rules distinguish pending matters from decided records and prescribe the particulars needed to trace the record. Before preparing a copy request, identify the court, case number, parties, year, order/judgment date and exact document required.

For the Gurgaon-specific workflow, use the Gurgaon Certified Copies guide. It explains how pending and decided records are routed and why a generic filing form should not be substituted for a copy application.

Forms or applications for court-file inspection

Inspection is also distinct from obtaining a copy. Punjab & Haryana High Court Rules and Orders contain specific subordinate-court rules for inspection of judicial records, including different treatment of pending and decided files and access by parties and non-parties. A request for inspection should therefore be framed for the inspection process rather than as a certified-copy application.

See the Gurgaon Court File Inspection guide for the court-specific route and the distinction between judicial-record inspection and other information-access mechanisms.

Five-layer form-readiness framework

LayerQuestion to answer before filing
1. ForumIs the document being filed in Gurgaon District Court, another District Court, the Punjab & Haryana High Court or another forum?
2. ProceedingIs it a new case, appeal, revision, interlocutory application, copy request, inspection request or another service?
3. Governing ruleWhich statute, procedural code, High Court rule or copying/inspection rule controls the format?
4. Filing channelDoes the current workflow use a downloadable form, online fields, uploaded PDF, physical submission or a combination?
5. Version checkIs the form current, complete and obtained from an official court/eCourts source?

This framework helps prevent a common filing problem: using a technically authentic form in the wrong forum or for the wrong procedural task.

A form does not replace the supporting document set

Even where a prescribed form exists, the filing may require accompanying pleadings, affidavits, vakalatnama or authority documents, annexures, indexes, copies, court-fee proof, identification of parties and other task-specific documents. The exact set depends on the nature of the proceeding and the current court rules.

For a broader filing sequence, use the Gurgaon Filing Procedure guide. If the submission is returned or marked defective, use the Gurgaon Filing Objections guide.

Forms to avoid using without verification

  • A form downloaded from a private document-sharing or template website when an official version is available.
  • A Delhi court form merely because Gurgaon is in the NCR.
  • A High Court opening sheet for a matter being instituted in the District Court unless the applicable rule specifically requires it.
  • An old saved PDF without checking whether the court or e-Filing workflow has changed.
  • A form with another district/court pre-filled unless the official source expressly uses a common statewide format.

Gurgaon court-form checklist

  • Identify the exact filing or service task.
  • Confirm the receiving court and case category.
  • Use the official eCourts, District Court or Punjab & Haryana High Court source as appropriate.
  • Check whether the current workflow is online, PDF-based, physical or mixed.
  • Confirm required signatures, verification, affidavit and annexures separately from the form itself.
  • Check court fee and process fee through the applicable fee workflow.
  • Keep the source URL and version/date of the form used for the filing record.
  • Re-check the form if the filing is returned for objections or if substantial time has passed.
